Position Overview:
Optimize patient care transitions and resource utilization in a premier academic medical center. Lead utilization review, discharge planning, and interdisciplinary coordination for high-acuity patients while ensuring compliance with EPIC documentation standards.

Key Responsibilities:
Conduct utilization reviews for appropriate care level/coverage
Coordinate discharge planning and post-acute care transitions
Document care coordination in EPIC
Ensure compliance with payer guidelines and regulations
Collaborate with ED teams for rapid case management
Analyze resource utilization trends and recommend improvements

Requirements
Qualifications:
Education: ADN/BSN from an accredited program
Licensure: Current and active New York State Registered Nurse (RN) license is required.
Certification: Basic Life Support (BLS) certification is required; candidates must hold a current certification or be willing to obtain recertification prior to employment.
Experience:
Must have Pediatric experience.
Minimum of 1.5 years of recent experience (within the last 2 years) in an acute care hospital setting of similar size and complexity.
Proven experience in hospital case management within an acute care environment is essential.
Technical Skills: Proficiency in the EPIC Electronic Medical Records (EMR) system is preferred.
