Cadence has been recognized by TIME as one of the Top 100 HealthTech Companies and by LinkedIn as a Top Startup (#4, 2025).</p></div><p class="isSelectedEnd"><span style="color: rgb(0, 0, 0);"><strong data-start="512" data-end="538">The Role</strong></span></p> <p class="isSelectedEnd">We're hiring a Nurse Practitioner to serve as the primary clinical point of contact for patients navigating their remote care journey.</p> <p>In this role, you will lead clinical decision-making with a strong emphasis on guideline-directed medical therapy for heart failure, as well as adherence to evidence-based guidelines for type 2 diabetes and hypertension, including proactive and asynchronous medication titration based on patient vitals, symptoms, and trends. You will oversee patients, manage clinical escalations, and collaborate closely with the patient’s physician or provider to optimize treatment plans, adjust medications, and determine the appropriate level of care. This role requires a high level of clinical autonomy, strong judgment, and close collaboration with Registered Nurses and Patient Success teams to deliver timely, patient-centered, and scalable remote care.</p> <p data-start="512" data-end="540"><span style="color: rgb(0, 0, 0);"><strong data-start="512" data-end="538">Licensure Requirements</strong></span></p> <ul class="p-rich_text_list p-rich_text_list__bullet p-rich_text_list--nested" data-stringify-type="unordered-list" data-list-tree="true" data-indent="0" data-border="0"> <li style="color: rgb(0, 0, 0);" data-stringify-indent="0" data-stringify-border="0"><span style="color: rgb(0, 0, 0);"><strong data-stringify-type="bold">Required:</strong> Active Compact Multi-State RN license</span></li> <li style="color: rgb(0, 0, 0);" data-stringify-indent="0" data-stringify-border="0"><span style="color: rgb(0, 0, 0);"><strong>Required:</strong> Active NP licensure in at least two of the following states: <strong>TX, MI, NC, CA, PA, NJ, CT, AL. </strong>Exceptional candidates with fewer licenses may be considered based on overall qualifications and ability to obtain additional priority state licenses.</span></li> </ul> <p><span style="color: rgb(0, 0, 0);"><em>Licenses must be active and in good standing at time of hire.</em></span></p> <p data-start="0" data-end="26"><span style="color: rgb(0, 0, 0);"><strong data-start="0" data-end="26">Schedule Requirements</strong></span></p> <ul> <li><span style="color: rgb(0, 0, 0);">Monday through Friday schedule (</span><span style="color: rgb(0, 0, 0);">Flexible scheduling available, with a core shift schedule of 9:00 AM to 6:00 PM in Mountain, or Pacific Time)</span></li> </ul> <p><span style="color: rgb(0, 0, 0);"><strong>What You'll Do<br></strong></span></p> <ul> <li>Oversee the monitoring of patient vitals, symptoms, and labs in collaboration with our team of RNs and MDs.</li> <li>Manage the patient's treatment plan and medications with an emphasis on initiating and titrating guideline-directed medical therapy (GDMT) for Heart Failure and other appropriate guidelines for Hypertension and Type 2 diabetes.</li> <li>Respond to patient escalations identified via RPM including abnormal vitals, symptoms, and labs.</li> <li>Lead regular virtual check-ins with patients to review labs and GDMT optimization and adherence.
