Engage Senior Therapy is seeking a dedicated Physical Therapist to join our team, delivering compassionate care at assisted living facilities in Concord, NH ! This is your chance to empower the geriatric and senior population to regain strength, mobility, and independence while working in a supportive, team-oriented environment.

As a Physical Therapist, you will:

• Evaluate patients to assess strength, balance, mobility, and functional impairments.

• Develop and implement personalized treatment plans that focus on improving mobility, reducing pain, and enhancing overall function.

• Deliver care at one or more assisted living locations, providing meaningful services to a diverse group of seniors.

• Provide therapeutic interventions such as manual therapy, therapeutic exercises, gait training, and balance programs.

• Promote safety and fall prevention by identifying risks and implementing preventive measures.

• Provide care with a high level of independence in assisted living communities, while always supported by the broader Engage team for guidance and resources.

• Educate patients and families on home exercise programs and strategies for maintaining functional independence.

• Document patient care services in compliance with state and facility standards using EMR.

• Enhance professional growth and development through participation in educational programs, in-service meetings, and workshops.

• One-on-one personalized patient care.

• Develop bonds not just with patients, but with their families, doctors, and fellow team members, ensuring collaborative care and successful outcomes.
