About this role
Cortica is looking for a full time Site Nurse Practitioner to join its growing team in Warren, NJ!
• In this role, you will conduct thorough patient intakes, screenings, and assessments, considering the unique needs of each patient and their family.
• You will also provide ongoing education to families and work closely with other specialists to offer comprehensive care, including nutrition evaluation, dietary support, and ongoing medical management.
• Work with our cognitive and behavioral child neurologists and pediatricians to provide ongoing care for attention, sleep, headaches, and challenging behavior, medication management as well as medical follow-ups.

What will you do?
• Complete thorough patient intakes, screenings, and assessments that incorporate the unique characteristics and needs of each patient and family member into the plan of care.
• Triage referrals for acute needs to other members of the Cortica team
• Collaborate with the interdisciplinary team members to coordinate compendium of care for each patient.
