What You’ll Do
As our Home Health Registered Nurse Clinical Manager, you’ll be the heartbeat of our clinical operations. Your days will be filled with purpose as you:
Support and mentor a dedicated team of nurses and caregivers
Ensure our patients receive safe, high-quality, personalized care
Oversee clinical documentation, care plans, and regulatory compliance
Collaborate with physicians, referral partners, and families
Lead case conferences and guide clinical decision-making
Help onboard, train, and inspire new team members
Keep a close eye on quality measures, patient satisfaction, and outcomes

Requirements
What You Bring
Active MI RN license
2–3 years of clinical experience in home health, hospice, or a related field is preferred.
Previous leadership or supervisory experience
Solid understanding of Medicare home health OASIS is a plus.
Comfort with EMR systems and clinical workflows
Experience working with physicians, referral partners, patient families, and interdisciplinary care teams
Bachelor of Science in Nursing (BSN) is a plus.
Experience with Kinnser is a plus.
Ability to work in the field as needed
