Private Duty Nurse (RN/LPN) – 1:1 Home Care

Job Summary

Centria Healthcare is hiring compassionate Registered Nurses (RNs) and Licensed Practical Nurses (LPNs) to provide 1:1 private duty nursing in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.

Client #1: Adult Client

• Location: Ypsilanti, MI

• Schedule: 8-Hour Night Shifts (10:00 PM – 6:00 AM)

• Days: Tuesday, Saturday

• Overview: Seeking a attentive nurse for a stable, home-bound adult client requiring ventilator, tracheostomy, G-tube, and seizure management. Supportive family is on-site to assist and provide orientation. Must be comfortable with small dogs (secured during shifts).

Client #2: Pediatric Client

• Location: Ypsilanti, MI

• Schedule: 12-Hour Shifts

• Days (8:00 AM – 8:00 PM): Sunday, Wednesday, Thursday, Saturday

• Nights (8:00 PM – 8:00 AM): Monday through Sunday (7 days/week)

• Overview: Join the dream team for a playful 1-year-old boy who loves dinosaurs! High-tech pediatric case requiring skilled trach, vent, and G-tube care. Family welcomes male and female nurses and offers both virtual and hospital-based meet-and-greets. Friendly dog in the home.

Client #3: Pediatric Client

• Location: Ypsilanti, MI

• Schedule: 12-Hour Shifts (9:00 AM – 9:00 PM)

• Availability: Monday through Sunday

• Overview: Rewarding opportunity caring for a sweet infant transitioning home from the NICU. Focuses on TPN administration, central line (Broviac) care, and G-tube management with zero respiratory care needed. Key tasks include TPN disconnects (late afternoon) and evening setups. Work alongside a supportive work-from-home mom in a lively, pet-loving household.

Responsibilities

• Provide skilled nursing care (trach/vent care, medication administration, catheter care)

• Monitor and document patient condition and vital signs

• Assist with daily living activities (ADLs)

• Communicate with families and care teams

• Maintain safe, high-quality, patient-centered care

Schedule & Pay

• Full-time and part-time shifts available

• Day, night, and weekend shifts

• Weekly pay + optional daily pay

Qualifications

• Active RN or LPN license in the state of Michigan

• Home care or private duty experience preferred (not required)

• Trach/vent experience a plus (training provided)

• Strong communication and critical thinking skills

Patient Population

• Pediatrics (birth–21 years)

• Adults with complex medical needs
