We're seeking a skilled and empathetic Full Time Hospice Case Manager to join our dynamic team in the Flint and surronding areas. This position offers a rewarding opportunity to make a significant impact in the lives of those we serve. Key Responsibilities: Conduct initial and ongoing assessments of patients’ needs to develop comprehensive care plans. Provide emotional support and education to patients and families about end-of-life care and self-care techniques. Collaborate with the interdisciplinary team to ensure high-quality care and regular updates on patient progress. Supervise and mentor nursing staff and paraprofessionals in delivering exceptional care. Maintain accurate clinical documentation and participate in performance improvement initiatives.

Requirements Current Michigan Registered Nurse (RN) License in good standing. Minimum of 1 year experience in hospice or palliative care. Strong assessment and critical thinking skills. Excellent communication and interpersonal skills. Ability to work collaboratively in a team environment. Empathetic and compassionate approach with patients and families. Willingness to participate in on-call duties as needed.
